Services We Provide
1. Community Support
Community Support Services provide an array of support services delivered by our community-based, mobile Community Support Specialist. The purpose/intent of a CSS is to provide specific, measurable, and individualized services to each client served. At 360 we provide CSS to children, adolescents and families based on the individual's diagnostic needs. Each client receives an individualized plan of measurable goals, that is delivered by our CSS. Community Support services should be focused on the person's ability to succeed in the community; to identify and access needed services; and to show improvement in school, work, family, and community participation.
2. Day Treatment Service
Day Treatment Services are mid-level mid level intensity programs to promote successful community living and well being for children and youth with serious emotional disturbance. Our Day Treatment is a structured treatment service for children and adolescents that build on strengths and addresses the identified needs. This medically necessary service addresses the beneficiary's diagnostic and clinical needs, which are evidenced by the presence of a diagnosable mental, behavioral, and/or emotional disturbance with symptoms and effects documented in a comprehensive clinical assessment and individualized plan.
Day Treatment is designed to serve children who, as a result of their mental health treatment needs, are unable to benefit from participation in academic or vocational services at a developmentally appropriate level in a traditional school or work setting. Our service will implement therapeutic interventions that are coordinated with the child's academic or vocational services available through enrollment in an educational setting. These interventions are designed to reduce symptoms, improve behavioral functioning, increase the individual's ability to cope with and relate to others, promote recovery, and enhance the child/adolescent capacity to function in an educational setting, or to be maintained in community based services.
3. Making A Plan MAP Teams
Making A Plan Teams address the needs of children/youth with serious emotional/behavioral disorders and an intellectual disability or serious disturbance and alcohol drug use; who require svc from multiple agency providers and multiple svc systems and who can successfully be diverted from inappropriate institutional placement. 360 inc, will participate on the local county MAP team which meets monthly.The MAP Team is composed of several local stakeholders in the targeted community. The goal of the team is to keep the child and youth population in their homes and communities and out of hospitals and institutions.
4. Peer Support Services
Our Peer Support services are individual interventions that focus on the consumers mental health. Our Certified Peer Support Specialist will assist children and youth in 360's treatment program to reach their expected goals/outcomes. The CPSS will assist the client in the program by modeling effective coping techniques, providing self-help strategies based on prior experience, supporting consumers in advocating for themselves to obtain effective services and helping them to monitor their progress. The CPSS is an integral part of the treatment team as they assist in the development of strengths-based individual goals, rehabilitation goals and develop community support.
5. Outpatient Therapy Services
Our Outpatient Therapy Svc is for individuals and families needing therapeutic help. Our Services focus on reducing psychiatric and behavioral symptoms in order to improve the beneficiary's functioning in familial, social, educational and occupational life domains. Our verifiable licensed staff will provide the aforementioned clinical services which are tailored to specifically meet the needs of our clients and families. Based on collaboration between the practitioner and beneficiary, and others as needed, the beneficiary's needs and preferences determine the treatment goals, frequency, and duration of services, as well as measurable and desirable outcomes.
6. Physician/Psychiatric Services
At 360, Psychiatric/Physician Services are of a medical nature provided by medically trained staff to address medical conditions related to the individual's mental illness or emotional disturbance. Medical services include medication evaluation and monitoring, nurse assessment, and medication management. All referrals will be evaluated to determine their need for Psychiatric/Physician services. This particular service is to provide the correct medication that supports therapy effects and removes barriers to maximize the benefit from therapy.
7. Crisis Response
Our Crisis Response services are available when there is an immediate need for compassionate and expert intervention. Crisis Response svc are available 24/7/365 to assist individuals who are experiencing a mental health crisis in their home, school or any other safe community location. A toll-free number will be available for crisis calls. Our Crisis response staff members are trained to evaluate and de-escalate crisis situations and provide the services needed to begin the process of recovery. Our team of professionals will deliver solution-focused and recovery-oriented behavioral health assessments and stabilization of crisis in the location where the individual is experiencing the crisis.To ensure that the services availability is publicized, we will have listings in the telephone directories for each county we serve and provide the information on our website.
